There are many possible reasons why couples may experience difficulties conceiving. In today’s society, many people delay starting a family due to education, career commitments, or personal goals. By the time they feel ready to have children, their most fertile years may already have passed.

In the majority of couples facing fertility challenges, an underlying medical issue is present in one or both partners, preventing a natural pregnancy.

In women, common causes include blocked fallopian tubes, endometriosis, and polycystic ovary syndrome (PCOS).

In men, fertility issues often stem from impaired sperm production or problems with sperm transport.

Additionally, lifestyle and environmental factors can reduce fertility in both men and women. These include prolonged stress, obesity, being underweight, excessive alcohol or nicotine consumption, drug use, intense exercise, physically demanding work, poor diet, and exposure to radiation. Past infections, such as mumps, can also negatively affect sperm quality.

Modern fertility medicine offers a range of treatment options, tailored to each couple’s individual circumstances. These include intrauterine insemination (IUI) and assisted reproductive technologies such as in vitro fertilisation (IVF), intracytoplasmic sperm injection (ICSI), and physiological ICSI (P-ICSI).

The fertility journey begins with an in-depth consultation.

This is followed by comprehensive diagnostic tests to identify the reasons for infertility. Based on the results, we develop a personalised treatment plan, which we discuss with you in detail. We then guide you through each stage of your treatment, providing continuous care and support throughout the process.

In most countries, fertility treatment is not considered a medical necessity and is therefore not covered by health insurance. However, in Austria, the IVF Fund was established in 2000 to assist couples with the costs of treatment. If certain criteria are met, the fund can cover up to 70% of the treatment costs.

Since 1 June 2018, Wunschkind Clinic has been a contracted partner of the Austrian IVF Fund.

A balanced and nutrient-rich diet can support the body’s reproductive functions and improve your overall health. In contrast, a poor diet lacking essential vitamins and minerals can negatively impact fertility.

Yes, moderate exercise is generally encouraged, as it supports physical and mental wellbeing. However, excessive or strenuous exercise can be harmful. If you regularly engage in high-intensity sports, consider reducing your training during fertility treatment. Gentle activities such as jogging, walking, swimming, or yoga are ideal.

Important note: The use of anabolic steroids or testosterone supplements can significantly reduce sperm production.

Fertility treatment is a joint journey, and partners should work together towards their shared goal of parenthood.

Men can contribute by maintaining a healthy diet, avoiding smoking, and reducing alcohol intake. Emotional support is equally important. Open communication, understanding, and patience can strengthen your relationship during this time. It is vital not to view infertility as personal failure. Seeking professional help together is a positive step towards achieving your goal.

Statistically, around 60% of couples conceive within six months of trying, and approximately 80% achieve pregnancy within a year. However, for some, conception takes longer or requires medical assistance.

Using your own eggs, IVF treatment is generally possible up to the age of 45, particularly if you have not previously had a child. Women who have already had children may still conceive beyond this age. Dr Brunbauer has successfully assisted women in achieving pregnancies at ages 47 and 48. Beyond this age, egg donation is usually the only option.

The World Health Organisation (WHO) advises that couples who have been trying to conceive for a year without success should seek medical advice.

If you are over 38, we recommend seeking help after six months of unsuccessful attempts. Fertility declines with age, so early intervention can increase your chances of success. Do not hesitate to contact our experienced team for support and guidance.

When should I go to a doctor

If  after one year of regular intercourse without contraception  pregnancy does not occur, you should seek medical advice. From then on the natural waiting time is exceeded. 

If you are  older than 38 years  of age, you shoud seek medical help  after  just  six months  to get to the root of the problem. 

There will be a stepwise diagnostic clarification. The  first step  is always a detailed  initial consultation. Among other things, we talk about  previous illnesses  and  treatments, current physical complaints  and your  sexual behavior. Then, depending on your individual situation, we will carry out various examinations. For example a tubal patency scan (HYCOSY). 

Infertility or fertility problems can have many causes. In women, it is often  hormonal problems  that cause ovarian dysfunction or problems with  egg cell maturation. The most common causes of fertility disorders or infertility in women are  occluded fallopian tubes, endometriosis  and the  PCO syndrome. Reasons for unfulfilled desire for children

Many causes lead to infertility or fertility problems. In women, it is often hormonal problems that cause ovarian dysfunction or egg cell maturation. However, it is not scientifically proven that tight pants alone lead to infertility.

Mumps is a childhood disease that usually occurs between the ages of five and nine years. Men who get mumps after puberty and develop testicular inflammation can reduce their fertility. In rare cases, fertility is lost.

Body and mind are one unit. If one part of this unit is affected, this also affects the other.

In the majority of couples, the unfulfilled desire to have a child causes physical causes. However, the interactions between body and psyche influence the situation. Endometriosis

The exact causes of this disease are not known. In endometriosis, tissue lesions develop in different parts of the body, which can cause cysts, adhesions, inflammation and scarring.

Endometriosis often causes severe menstrual pain, pain before, during and after sex, bleeding disorders in the form of very long or very heavy periods, lower abdominal pain, nausea and malaise. The disease is often accompanied by an increased susceptibility to infection. Lack of drive, mild fever or back pain are also among the symptoms.

Endometriosis mainly affects women in the period between their first menstrual period and menopause. 10 to 15 percent of all women in this age range have endometriosis. That is about 300,000 women in Austria.

The disease is clearly diagnosed by surgery, usually with laparoscopy. If you think you are affected, consult your gynecologist or come directly to us at the Wunschkind Clinic. Your suspicions will be checked with a palpation examination and an ultrasound.

Since endometriosis cannot be cured, therapies aim to relieve the symptoms and slow down the progression of the disease. Surgery can remove the endometriosis lesions. Hormone therapies are used to suppress symptoms. Clarify which therapy is most promising for you.

40% of women affected by endometriosis do not get pregnant naturally because of the disease. Get certain about your situation if you have endometriosis and wish to have a baby. Also, if you are affected by infertility due to endometriosis, you have several options to still get pregnant. Depending on your initial situation, different options open up: Hormone treatment, insemination or one of the two forms of artificial insemination. We will examine you in detail and then recommend the therapy that is best for you.

PCO SYNDROME

Only a doctor can definitively determine this. Various examinations are necessary for this, such as a survey of your hormone status and a vaginal ultrasound. If you notice the typical symptoms of a PCO syndrome in yourself, go to a trusted doctor for diagnosis or come directly to us. PCO syndrome produces symptoms that are typical of other diseases. To be sure, a medical clarification is essential.

Women suffering from PCOS usually complain of an irregular menstrual cycle, severe menstrual cramps, intense body hair including on the face, obesity, oily skin or acne, and sleep disturbances

The exact cause is as yet unknown. Treatment of the disease is aimed at alleviating the symptoms and limiting secondary symptoms such as diabetes.

The disease often affects the body's sugar balance. As a result, insulin resistance can develop. It is therefore important to clarify quickly whether your complaints are caused by PCO syndrome and to initiate therapeutic measures.

Depending on the individual situation, various treatment options are available, for example: nutritional supplements, medication or surgery. If there is a desire to have children, this can be fulfilled by artificial insemination if the other therapies do not successfully lead to pregnancy.

Yes, the chances are even good. It is important that you take advantage of comprehensive medical support so that your therapy proceeds in a controlled manner and all risk factors are minimized as far as possible.

Pregnant PCO patients are more frequently affected by complications such as miscarriages, gestational diabetes or eclampsia (pregnancy poisoning). However, with careful medical support, you can keep these risk factors under control so that you can have the baby you want with as few complications as possible.

Pregnant at 40

In principle, it is possible to conceive at this age. In addition to age, the probability is influenced by other factors such as medical history and your lifestyle. 

Every woman is born with a fixed number of eggs. The number decreases with age, and the egg reserve becomes smaller. Also, the quality of the eggs decreases as the woman gets older. In addition, with increasing age, other risk factors can affect fertility: diseases, stress, being overweight or underweight, mental stress, excessive exercise or alcohol or nicotine consumption, and a diet low in vitamins. Nevertheless, women over 40 have a chance of conceiving. 

The rule is: the younger, the better. For a 20-year-old woman, the chance of pregnancy within one year is 86%, for a 40-year-old it is 36%.

Yes, pregnancy at this age is classified as a high-risk pregnancy. However, this does not automatically mean that you have to expect increased complications. Your pregnancy will be monitored more closely so that, if possible, nothing stands in the way of you having your baby. 

This varies from woman to woman. In general, fertility decreases at this age. If you have regular unprotected sexual intercourse for half a year and no pregnancy occurs, clarify the causes and ask about treatment options suitable for you. Come to us - we will be happy to help you. 

If pregnancy does not occur naturally, the first step is to find the cause. Depending on your personal situation, we can then carry out hormone treatment, insemination or in vitro fertilization with your own or donated eggs.

In the initial interview we get to know each other, we take your detailed medical history and develop a strategy for your further treatment. That depends on your individual situation.  

Man and woman are included in the diagnosis. We discuss all necessary examinations with you in detail. Possible investigations include an ultrasound examination of the woman, a female hormone panel, examination of the tubal patency (HyCoSy) and a semen test  (spermiogram)  in men. 

As a rule, we conduct an  ultrasound examination  on the woman during the  initial consultation  in order to gain insight into your specific situation from a fertility perspective. 

The so-called Hydro Contrast Sonography (HyCoSy) is an almost risk-free  and  gentle examination. It takes about five to ten minutes. It may cause mild, menstrual-type pain that quickly passes. 

No, the examination is performed on an outpatient basis without anesthesia.

We examine the spermogram quickly. Usually you get the results after 30 minutes.

In principle, this would be possible, but we prefer the semen sample to be done at the Wunschkind-Klinik. We provide you with a  discreet space in which your privacy is maintained. The main reason is that no more than 60 minutes should elapse between ejaculation and examination of the sperm. The  temperature  also plays an important role. In addition, only specific containers may be used to prevent contamination of the semen sample. 

In the clinic, the optimal conditions are met to enable accurate semen analysis.

We use a spermiogram to assess male fertility. In doing so, we evaluate the quality of your sperm according to various criteria. These include the number of sperm in the ejaculate, the shape, motility, liquefaction time, volume of the ejaculate, viscosity (whether the semen sample is viscous), pH, color and vitality (how many live sperm are contained in the ejaculate).

First, we will inform you in detail and make an appointment with you. You should be sexually abstinent for at least three days before this date. You should not have a cold, inflammation or fever on the day of the spermiogram, as this may affect the result. In our clinic, we have a discreet room where you will obtain the semen sample for us to examine by maturbarion. We will discuss the results and subsequent treatment recommendations with you and your partner in a follow-up appointment. 

If the result of your spermiogram is not due to a previous disease or has genetic reasons, you should consider lifestyle changes. Smoking, drinking large amounts of alcohol, being overweight and excessive stress have a negative effect on sperm quality. Healthy diet, regular exercise and emotional well-being help to improve sperm quality. 

A complete sperm production cycle takes 96 days. In principle, a spermiogram can be done about 12 weeks after cancer treatment. However, the regeneration of fertility depends on various factors, such as the level of the treatment dose received or the age of the patient. Often, the spermiogram was already of reduced quality before the start cancer treatment. Seek advice from your treating physician before scheduling a spermiogram. Especially in the case of atesticular tumor, full recovery can take up to three years, and in certain cases up to nine years.

Artificial Insemination (IVF)

We offer you these types of artificial insemination:

  • IVF (In Vitro Fertilisation)
  • ICSI (Intracytoplasmic Sperm Injection)
  • P-ICSI (Physiological ICSI)

In vitro fertilization involves removing the egg from the ovary and then placing it in the test tube with the man's spermFertilization in the test tube takes place naturally, without medical intervention. If the fertilization works, we put the resulting embryo into the uterus two to five days later.

In detail, an IVF usually runs according to this scheme:

  1. Hormone treatment
  2. Initiation of ovulation
  3. Taking the egg
  4. Fertilization in the laboratory
  5. Transfer of embryos to the uterus
  6. Success control

The chance of getting pregnant through IVF treatment is 50% per cycle. The birth rate is 35% per treatment course.

The success depends not only on the fact that the treatment itself is uncomplicated. The probability of pregnancy is also influenced by the age of the couple, the hormonal situation and the lifestyle. Obesity, stress, nicotine and excessive consumption of alcohol can reduce the chance of success.

ICSI (Intracytoplasmic Sperm Injection) is the most common fertility treatment. It helps especially couples, in which the fertility of the man is severely limited.

The course of an ICSI corresponds for the most part to that of in vitro fertilization. However, in an ICSI, a single spermatozoa is injected directly into one of the ova with a needle. This increases the probability of fertilization. If the fertilization is successful, the resulting embryo is transferred to the woman's uterus on the second to sixth day after taking the egg. The further procedure corresponds to that with an IVF treatment.

The prospects of ICSI are similar to those of IVF. The pregnancy rate is 50% per cycle. The birth rate is 35% per treatment course.

In a P-ICSI (Physiological ICSI) the spermatozoa are selected for the ICSI.

Mature sperm are separated from immature by a hyaluronic acid pre-test. In mature sperm hyaluron is a fixed component of their shell. Immature sperm do not have this component. Using this procedure we select a mature sperm cell under the microscope for ICSI treatment.

Apart from the selection of sperm used, a P-ICSI works the same way as an ICSI.

Yes. Fertility declines with age. The best age range for a first child is 25–35, when the body is most resilient. The younger you are, the higher the chances of success.

If more embryos are created than needed, they can be frozen (cryopreserved) for future cycles.

This often eliminates the need for another round of hormone stimulation.

If you are unwanted childless, it is essential to clarify the causes. We carry out detailed examinations and recommend the appropriate treatment based on your diagnostic results. This may include IVF. In some cases, however, simpler therapies are sufficient to achieve pregnancy.

Austrian health insurance does not cover IVF. However, the IVF Fund covers 70% of the cost if eligibility criteria are met.

The IVF Fund covers up to four cycles per couple. If you’re self-funding, there’s no set limit.

Yes. You must either be in a registered partnership or provide a notarised declaration confirming your consent and understanding of the legal implications.

A typical IVF cycle lasts several weeks, depending on your cycle. It’s carefully timed to optimise your chances of success.

Hormonal Treatments

The three most common hormone disorders that occur in the absence of pregnancy are the PCO syndromeendometriosis and yellow body weakness.

Depending on your individual situation, we use hormone treatment to pursue one of these three goals:

  • Stabilize your menstrual cycle.
  • Stimulate the maturation of oocytes.
  • cause an ovulation.

The hormone treatment is done with tablets or with syringes that you or your partner can administer to you. During the entire treatment period, we observe attentively the maturing of the egg. So we can determine the exact time of ovulation.

About one in three women becomes pregnant naturally after receiving hormone treatment and knowing the best time. Sometimesseveral treatments are necessary until the wish for a child is fulfilled.

Hormone therapies can have unpleasant side effects. In some patients, the treatment leads to hot flashesdizziness or blurred vision. In rare cases overstimulation occurs.

We make sure that hormone therapy is well-dosed. In doing so, we act according to the motto "as much as necessary, as little as possible", so that the burden on you is kept within limits as far as possible.

Even in men, the hormone balance can get out of whack. Especially the sex hormone testosterone plays an important role in sperm production.

 A hormone therapy can provide relief and contribute significantly to the success of having children.

Depending on the individual situation, we carry out the appropriate therapy:

  • Capsules taken daily
  • injections
  • Administration of other hormones that stimulate semen production

Insemination

At the most fertile time in your cycle, sperm is placed directly into the uterus to increase the chance of fertilisation.

In these situations, we advise you to semen transfer:

  • Low sperm count or poor motility
  • Erectile or ejaculation problems
  • Issues with sperm reaching the uterus
  • Cervical mucus blocking sperm
  • Physical conditions preventing intercourse

Yes, though minimal. Risks may include:

  • Hormonal side effects (e.g. mood changes)
  • Mild infections
  • Psychological stress
  • Possibility of multiple pregnancies

 

Pregnancy rates are between 12–15% per cycle. Lesbian couples often see slightly higher success rates due to fewer underlying reproductive issues like PCOS or blocked fallopian tubes.

It varies. Most couples require several attempts.

Naturally – after insemination, the sperm travels to the fallopian tubes, where it may meet and fertilise an egg.

We wait. A pregnancy test is done about three weeks later. If successful, a heartbeat can usually be seen three weeks after that.

Flexibility is important. Insemination is timed precisely to the woman’s cycle. Ideally, fresh sperm is used and collected at our clinic.

EGG donation

Yes, it has been legal since 2015. 

Usually yes. Unless the couple brings their own donor. 

No. Compensation for egg donation is prohibited

No. The donor has no legal or financial obligations. The child also has no inheritance rights.

No, they are not legally permitted. 

Yes, sperm donation is legal and can be used as part of IVF treatment. Both partners must provide notarised consent. 

In donor egg IVF, the hormonal stimulation is carried out on the donor. The rest of the process is the same. 

Hormonal stimulation of the donor takes around two weeks. After retrieval, the embryos are fertilised and frozen. The entire process takes about three weeks

Preimplantation diagnostiCS

With the pre-implantation diagnosis, we clarify whether your eggs or embryos are healthy. It allows us to select specific unencumbered oocytes or embryos and to avoid genetic defects in your baby.

PID can be carried out on oocytes and embryos.

Precondition for the pre-implantation diagnosis is that an artificial insemination (IVF, ICSI) takes place. The examination takes place outside the body, even before we transfer the embryo into the uterus.

We differentiate in polar body diagnosis (PCD) and trophectodermbiopsy (TOBI). In polar body diagnosis, we clarify whether genetic disorders, genetic defects or structural chromosome disorders are present in women. The Trophektodermbiopsie is primarily used when a miscarriage or stillbirth or a specific hereditary disease should be avoided.

The investigations serve to select the best genetically suitable embryos. This increases the likelihood that the embryo is healthy.

Yes, because the most genetically suitable and fittest embryos are selected.

Cryopreservation

Spermoocytes and embryos can be cryopreserved.

Embryos can be stored in Austria for ten years.

Unfertilized ova may be kept for life.

For spermlifelong storage is allowed.

In sperm, about half survive freezing and later thawing. The remaining amount is usually sufficient to fertilize eggs in the course of an IVF or ICSI.

The cryopreservation of embryos works largely lossless. Up to 95% of the frozen embryos can be thawed vital again.

In 2016, Chinese scientists found that the "baby-take-home-rate" after artificial fertilization with frozen embryos is higher than with fresh ones.

Social egg freezing means that women freeze young ova as reproductive measures. Egg freezing is allowed in many countries.

In Austria, oocytes may only be removed and stored if there is a risk that their reproductive capacity will be lost as a result of a physical illness or therapy (eg chemotherapy).

Ovarian cells should not be cryopreserved for reasons of simpler family planning.

Eggfreezing

Egg freezing and social egg freezing allow young women to freeze their eggs making them available for future in vitro fertilization. Thus the wish to have a baby can be realized even at an advanced maternal age. Egg freezing gives women the option to postpone their family planning. 

However, social egg freezing is prohibited in Austria

In  Austria, oocytes  may only be retrieved and stored if there is a risk of  losing the ability to reproduce  due to a physical condition or medical treatment (e.g. chemotherapy). 

Oocytes  may not be cryopreserved merely for  reasons of simpler family planning.
